Medical Coding is the transformation of healthcare diagnosis and their procedures into universal medical alphanumerical codes.The main task of a coder is that to review the patient's clinical records and statement and then convert them to codes for quality care in future visit.Medical Coding is performed all around the world using the International Classification of Diseases (ICD). ICD is maintained by World Health Organisation  (WHO) and modified by each member country to serve its need.

While complex and detail-driven,medical coding really comes down to knowing how to navigate the three main code sets: CPT (Current Procedure Terminology), ICD (International Classification of Diseases) and HCPCS (Health Care Procedural Coding System).These code sets help coders to document the condition of a patient and describe the medical procedure performed on that patient in response to their condition.

The Medical Coders ensures the codes are applied correctly during the medical billing process. This involves abstracting the information from documentation, assigning the appropriate codes, and generating a claim to be paid insurance providers. A record of a patient's visit, not only benefits the physician, it's how they get paid. Medical codes translate the documentation into standardized codes. The coder also checks the patient's medical record, such as the transcription of doctors' notes, requested image studies , ordered laboratory tests etc to demonstrate the work that was completed.

Coding requires extraordinary detail. The provider must carefully review the patient's chart to learn the diagnosis and itemize every service provided. If a service is overlooked, the provider will not receive payment for it. If the coder chooses the wrong code the provider may have to return any excess payment or face legal charges for overbilling.
